Title        Community Health Worker –Youth Program Focus
Classification & Compensation Full -Time 

$20-$23 per hour (non-exempt)

Location: WellShare Headquarters, Minneapolis, MN 

POSITION SUMMARY 

The Community Health Worker (CHW) is responsible for implementing components of WellShare programs, primarily related to work engaging Somali/East African Youth in after-school setting; engaging Somali/Somali American community members in the Twin Cities area and beyond on predetermined health topics. The CHW ensures high quality standards and performance in the delivery of CHW services, internal and external communication and collaboration, community outreach, data management, education, and research. The CHW ensures that activities are in line with WellShare’s mission, vision, and values. The CHW must be a self-motivated, adaptable, relational leader and team-player to ensure that the organization achieves strategic goals.  

RESPONSIBILITIES 

Program Implementation, Monitoring and Reporting 

  • Participate in strategies, implementation plans, and educational tools as part of a team. 
  • Monitor the quality and timeliness of program implementation for continuous improvement.
  • In coordination with program coordinator and manager, support development of and implementation of work plans and timelines to achieve program goals/objectives as a member of a team 
  • Follow detailed implementation plans, program strategies, and curricula or other educational tools with Supervisor
  • Adapt programming based on results of evaluation to support program and stakeholder goals, after consultation with Supervisor 
  • Adhere to reporting requirements as outlined by donor agencies, with support from the Supervisor (Program Manager/Director) 
  • Provide data quality control within various electronic health record systems.
  • b) Monitor the day-to-day delivery of the programs and services for each assigned grant 
  • Maintain accurate and organized program records 
  • Track progress of planned program activities against proposed timelines

Communicate and collaborate with internal and external stakeholders

  • Collaborate with Supervisor to ensure team members’ concerns, challenges, and successes are noted and responded to as needed
  • Support overall logistics of program activities and enabling transparency and accountability for all stakeholders 
  • Establish and maintain strong working relationships with community groups, funders, and partner organizations. 
  • Represent the organization at community activities to enhance the organization’s community profile

Conduct health outreach, education, system navigation support and research

  • Provide direct service to individuals and groups with high standards of care and trust upholding HIPPA requirements
  • Coordinate clinic and/or social service referrals as relevant
  • Plan, promote and conduct culturally appropriate community health events in collaboration with other staff and Supervisor. 
  • Implement health communication activities using social and broadcast media. 
  • Contribute to the design and implementation of research and evaluation.
  • Implement work and ensure program deliverables are meeting set goals

Track Program Assets/Technology

Facilitate youth program session meetings

  • prepare weekly lesson plans and lead delivery of sessions

Other

  • In consultation with Supervisor, pursue new, strategic partnerships with community-based health organizations, health plans, and other relevant agencies. 
  • Contribute to grant writing, narrative and financial proposals.
  • Other duties and activities as requested and being available as needed evening and weekend

QUALIFICATIONS 

Minimum Required Education, Experience, and Knowledge 

  • Associate of Arts Degree with a minimum two years of experience the field; or bachelor’s degree in public health or related, OR equivalent education/work experience
  • Ability to speak, read, and write in both Somali and English at a professional level ∙ Experience designing, implementing, and evaluating community-based health programming – particularly with Somali/Somali American males 
  • Experience working with diverse communities (i.e. experience conducting focus groups and/or key informant interviews) and analyzing results 
  • Ability to competently use computers and related software including Microsoft Office tools, Google applications, and other internet-based tools 

Desired Education and Experience 

  • Community Health Worker certificate 
  • A minimum of two years of experience in positions related to public health 
  • Experience working in refugee or immigrant communities, or working internationally 
  • Experience collaborating with a team of diverse employees 

KNOWLEDGE, SKILLS, AND PHYSICAL REQUIREMENTS

  • Fluent in Somali and English. 
  • Excellent interpersonal communication skills to effectively develop relationships, interact, and collaborate professionally and positively, with a diverse group of individuals inside and outside of the organization
  • Effective organizational, multi-tasking, and time management skills
  • Self-motivated and detail-oriented, able to work independently
  • Ability to sit at a desk for extended periods with efficient computer keyboarding skills
  • Visually review detailed reports and information; hear and speak by telephone and online meeting platforms
  • Current valid driver’s license and reliable, insured vehicle as required by State law
