Community Health Worker – (Spanish speaking) – CERTIFIED

Using a community health worker (CHW) model, we work with underserved communities in Minnesota and rural Africa. Our programs serve people across the lifespan – including pregnant women, infants, children, youth, adults, and the elderly.

Position Description:

WellShare Community Health Workers (CHW) are the heart of our team’s work to provide individual and group health education, assist people with health system navigation and social service referrals to support people’s ability to live the healthiest life possible.  Work is conducted primarily during regular work-day hours, but some evening and weekend hours may be available or required. Work is predominantly remote with occasional visits to the Minneapolis WellShare office.  Some projects include regular travel for client engagement around the Twin Cities.

Responsibilities:

  • Program implementation:
    • Assist clients in identifying overall health/life needs and set goals
    • Provide health education and coaching to clients in individual and group settings
    • Provide referrals for primary and behavioral health care and social services
    • Recruit/intake and maintain participants for programming
  • Administration:
    • Maintain accurate and organized program records
    • Provide clear and appropriate oral and/or written communication to partners
    • Work cooperatively with partners and stakeholders
    • Adhere to reporting requirements as outlined by donor agencies
    • Some programs require electronic health record data-entry
  • Provide content and technical assistance:
    • Contribute to the creation of education and outreach materials in print, video and social media.
  • Perform additional duties as assigned

Qualifications:

  • CHW Certificate
  • Passion for assisting individuals to meet their health goals
  • Somali, Spanish, Hmong or Karen language fluency
  • Excellent oral and written skills in English
  • 1 year of experience in health education and community outreach preferred
  • Ability to use computers and related software including word processing, spreadsheets, basic database management, and internet-based tools
  • Experience working with underserved populations
  • Effective interpersonal communication skills
  • Ability to work collaboratively with others in program planning and implementation
  • Self-starter with ability to juggle multiple priorities and meet deadlines

Physical Requirements:

  • Use the telephone and communicate with people both in and out of the office environment
  • Sit at a desk for extended periods and travel to multiple locations within a day
  • Manual dexterity for computer keyboarding and office equipment use
  • Possess visual acuity to review detailed reports and information
  • Must have reliable transportation and a valid driver’s license

Employment Status:  Part Time and Full-time positions available, non-exempt

Reports To:  Program Manager

Compensation: $200 hiring bonus, $18 – $24/per hour, generous benefits (3 weeks PTO, 401K match, health, vision, dental insurance)
